Seniors and Persons with Disabilities

The Home and Community Care Programs help people live in the community as independently as possible. Our services can help the elderly and people with disabilities, and their families, with service planning and getting the help they need.

Paying for Services

During an assessment visit, our assessors will discuss insurance or other payment sources which could help with the cost of services. Some people are eligible for assistance with part or all of the cost of services through state and federal-funded programs administered through Chippewa County, such as Alternative Care (AC), Elderly Waiver (EW), Community Access for Disability Inclusion Waiver (CADI), Community Alternative Care (CAC) and Brain Injury Waiver (BI). Private pay options can also be arranged for eligible services.

Community Alternative Care Waiver (CAC)

The Community Alternative Care Waiver is designed to support chronically ill children and adults, who would otherwise require hospital-level of care, by providing supports and services that will help them live successfully in the community.

Community Access for Disability Inclusion (CADI)

The Community Access for Disability Inclusion (CADI) is a home and community-based waiver that provides funding for persons under the age of 65, with a certified disability, on Medical Assistance, meets the nursing facility-level of care criteria, and needs supports and service that will help them avoid or delay institutional living.

Developmental/Intellectual Disabilities (DD/ID)

The Developmental Disability Waiver provides funding for home and community-based services for children and adults with intellectual and developmental disabilities. Waiver service may be provided in a person’s home, in a relative’s home, or in a foster care home.

Personal Care Assistance Program (PCA)

The Personal Care Assistance (PCA) Program provides services to people who need help with day-to-day activities to allow them to be more independent in their own homes. A personal care assistant (PCA) is trained to help with basic daily routines. A PCA may be able to help you if you have a physical, emotional, or mental disability, a chronic illness or an injury.

Home Care Services

Home Care Services provides medical and health-related services and assistance with day-to-day activities to people in their home. It can be used to provide short-term care for people moving from a hospital or nursing home back to their home, or it can also be used to provide continuing care to people with ongoing needs. 

Brain Injury Waiver (BI)

The Brain Injury (BI) Waiver provides funding for home and community-based services for certified disabled individual, under the age of 65, with an acquired brain injury, on Medical Assistance, and who require community support in order to avoid or delay nursing facility or neurobehavioral hospital placement.

Alternative Care (AC) Program

The Alternative Care (AC) Program is for people over the age of 65, with low income and assets, who are not eligible for Medical Assistance. This program provides financial assistance to seniors and/or their families for home care services to help older persons remain in their home.

Elderly Waiver (EW) Program

The Elderly Waiver (EW) Program promotes community living and independence with services and supports that address each person’s individual needs and choice. The Elderly Waiver is for people over the age of 65, who are on Medical Assistance, and have an assessed need for supports and services that will help them avoid or delay institutional care.
